PLEASE NOTE: Members who are covered for Term Life Insurance are automatically enrolled for Dependent Term Life Insurance for their spouse and dependent children. $1.24 per month covers all your dependent family members. Each dependent family member is covered for a life benefit of $5,000.

Important information about your premium rates:

Premiums are calculated based on your age and applicable rate as of January 1 each year. As the rates are in 5-year age bands, premiums will increase as you reach a higher age band.

**Non-smoker rates apply to individuals who, at the time of application, have not used tobacco, nicotine, or cannabis mixed with tobacco in any form whatsoever within the last 12 months and who have provided satisfactory evidence of insurability.

† On the January 1 following your 60th birthday, the amount of Voluntary Group Term Life Insurance benefit in force will reduce to the lesser of $250,000 and 50% of the amount in force at age 59.
