Important Information
All Members of ALPA Canada Insurance Trust, and their spouses are eligible to apply. Member means all regular active, executive active, executive inactive, furloughed, grievance pending, inactive participant, military leave, personal leave, retired and sick inactive members in good standing with the Air Line Pilots Association, holding a valid Category 1 or Class 1 medical report by a Civil Aviation Medical Examiner and insured under a provincial or territorial government health insurance plan. Applicants must be under age 65 and reside in Canada. Your spouse can apply, even if you choose not to.
Note: If you already have $75,000 of Guaranteed Acceptance Term Life Insurance under this plan you are not eligible for this offer. If you already have $50,000 of Guaranteed Acceptance Term Life Insurance you may only apply for an additional $25,000. If you already have $25,000 of Guaranteed Acceptance Term Life Insurance you may only apply for an additional $50,000.
Spouses must meet the policy definition for spouse.The only exclusion is suicide occurring within two years of the effective date of your insurance.
Coverage terminates on the earliest of the following dates:
- the end of the year
following the date you reach age 70 ; - the end of the month in which your written request to cancel your insurance is received;
- the date the Group Master Policy between ALPA Canada Insurance Trust and iA Financial Group terminates;
- with regard to your spouse's Term Life Insurance, the earliest of the above or the end of the policy year following the date they reach age 70
, or the end of the month in which they no longer qualify as a "spouse".
Spouse means the legal spouse of an eligible member/employee, providing there is no separation in effect, or an individual who has been residing in a marriage-like relationship with the member/employee for a period of at least 12 consecutive months.
Insurance will take effect on the date your completed application is received by Industrial Alliance Insurance and Financial Services Inc. and your first month's premium has been paid.
If you no longer have active employment, or if required, active membership, or an insured spouse or dependent child(ren) no longer meet(s) the eligibility requirement under this group policy, you should contact our Client Service line on 1-800-266-5667. A Client Service Team Member will walk you through the necessary steps. Termination requests must be submitted in writing.
When our office is advised of your ineligibility (in writing) within 3 months of the ineligibility date, you may be entitled to a 3 month premium refund.
